MIDDLE SCHOOL BAND INTENT FORM

In order to organize a band program, certain responsibilities need to be assumed by the students and parents:

  1. Each student must attend all scheduled rehearsals and performances.  Parents should contact the band director a full week ahead of a scheduled performance/rehearsal if the student will not be present.
  2. Fundraising is a vital part of the financial operation of the band program and all students must participate.
  3. Students must have his/her instrument, book, music, pencil, supplies, and other required items every day for class.
  4. Students must practice 25 minutes per day and have music worked up to performance level at the assigned time.
  5. Students must complete their playing requirements to be promoted to the next performing level.
  6. All parents are members of the Band Boosters organization and encouraged to participate.


NEGLECT ON PART OF THE STUDENT/PARENT TO FULFILL THE ABOVE WILL RESULT IN LOSS OF GRADE POINTS.



I have read and considered the guidelines listed above involving participation in the band program.  I understand my responsibilities to the band and hereby make the commitment to be an active part of the band program.
